In consideration of accepting this entry, I hereby, on my own behalf and on behalf of my heirs, executors and/or administrators, waive and release any rights and claims and damages of loss I may have against the hosts, its employees, insurance carriers, sponsors, and tournament. I agree that the tournament officials and sponsors shall have exclusive rights to photos, videos, and audio recording of myself, and may use or dispose of such materials in any lawful manner. I further agree to submit to a polygraph test upon request by tournament officials. In signing this application, I certify that I have liability insurance providing coverage for personnel injury or property damage that might occur on my behalf.
